Bluetails Pet Rescue Adoption Application

Please answer all questions completely and thoroughly. This includes landlord name & phone number & veterinarian name & phone number. We also recommend that you contact your landlord and vet and grant them permission to disclose your information to us. If you purchased heart-worm prevention someplace other than your veterinarian, you will be required to show proof. If we need to request additional information or wait for permission to be granted, we will move on to the next applicant while we are waiting for your information.


We strictly adhere to our adoption process as it is designed to find the best possible placement for each homeless animal in rescue. The basic steps are described on Our Adoption Process Page.

Where will you primarily keep this pet?
Please note, that fenced yards are required for puppies, young, most adult, some senior dogs, high energy breeds, large breeds, prey-drive breeds, dogs with anxiety and other possible reasons. We do, however, have leash appropriate dogs for partially or unfenced yards that our coordinators can suggest for you.
Are all the members of your household in agreement about adopting a dog?
Does anyone in the household have pet allergies?

Are you prepared to assume the financial responsibility of caring for this pet? Including, but not limited to, vaccines, annual vet visits, quality pet food, monthly preventions, grooming and training that could average over $1000/a year?
If you are not ready just yet, we can get your pre-approved but we can not hold a pet.

Tell us about your current & past pets
Are your current pets or were your past pets up to date on their vaccinations and consistent heartworm preventatives? This includes either oral prevention or injectable prevention. We will verify this information with your vet.
Have they been or were they vaccinated yearly? We will verify consistency with your vet. If we can not establish consistent vaccinations or heartworm prevention you may be processed into our Foster to Adopt program.
Have they been or were they on heartworm prevention consistently? We will require proof that consistent prevention was purchased. If prevention was acquired from somewhere other than your vet, please list those sources here. If we can not establish consistent vaccinations or heartworm prevention you may be processed into our Foster to Adopt program.
Has your current or past pets been spayed or neutered? Are you interested in receiving information about low cost spay and neuter programs?
If you currently have pets, are they good with other dogs?
Are you willing to work through unexpected hardships with your pet and hire a trainer if need be?
Are you willing to allow a representative of Bluetails to visit your home?
Are you willing to allow Bluetails to perform periodic follow-ups?
If you are no longer able to keep this pet, do you agree to return the pet to Bluetails?

Our Approach is Simple...

Our approach to rescue is at its core fundamentally different than conventional methods. Bluetails is a non-profit, volunteer animal rescue organization, but unlike other groups we receive the majority of our funding through providing Pet Care Services throughout the Greater New Orleans area. However, like other rescue organizations, we do graciously accept donations.


We are dedicated to rescuing dogs left homeless for whatever reason – animals in public shelters where they are at high risk of euthanasia due to pet overpopulation, pets given up by their owners because of difficult circumstances, and those in danger of abuse or neglect. ~ Courtney Jean Davis
